POSITION SUMMARY:
Reporting to the Site Administrator, the Licensed Practical Nurse (LPN) is responsible and accountable for providing optimal care and support to clients, clients families and volunteers using a harm reduction approach. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the CLPNA, and according to organizational policies, procedures, standards and unit specific protocols.
KEY ACCOUNTABILITIES (INCLUDE BUT NOT LIMITED TO):
- Responsible for maintaining a collaborative and open environment with Alberta Health Services Case Managers (AHSCM) as well as the Interdisciplinary Team.
- Lead and maintain all nursing processes in accordance to TPFA policies and procedures as well as CCHSS standards and AB Accommodation Standards.
- Provide direction and supervision to Health Care Aide staff as per facility policies and procedures.
- Provide excellent care services to clients by being extremely responsive to the needs and wants of clients by adhering to the care plan, as well as facility policies and procedures.
- Respond to client/family concerns and ensure appropriate action taken and or concern communicated to the Client Services Manager and other Departmental Heads.
- Perform a through nursing assessment of the client to establish and allocate appropriate resources through collaboration with the Interdisciplinary Team.
- Utilize TPFA policies and procedures, document client care assessments, interventions, and outcomes to ensure accurate capture of client clinical and mental status.
- Document timely, accurate reports of data collection, interpretation, planning, implementing, and evaluating care in accordance with TPFA policy, and regulatory requirements.
- In Collaboration with the Alberta Health Services Case Managers (AHSCM) and the Interdisciplinary Team, schedule and lead the clients care conference with family and clients when applicable.
- Work closely with the Client Services Manager and the Alberta Health Services Case Managers (AHSCM) in the planning of all client admission, transfers, and discharges.
- Provide direct client care and medication/treatment administration and document care in compliance with regulatory and TPFA standards, policies, and procedure as well as CLPNA standards.
- Processes the orders and safe keep drugs and narcotics and administer in compliance with all regulatory requirements, scope of practice and TPFA policies and procedures.
- Plan, coordinate, manage & evaluate the client's care plan working with the client/substitute decision maker, physicians, nursing and interdisciplinary team and contracted service providers.
- Coordinate nursing teams client care assignments to ensure efficient and timely delivery of services to clients.
- Support the Client Service Manager in the coordination, implementation and monitoring of Infection Prevention and Control, Occupational Health and Safety, Quality Initiatives for the Nursing Department.
- Complete audits on Point of Care tasks to check for timely completion of documentation.
- Coordinate and serve on the various committees responsible for client care.
- Complete Annual & Mandatory Education/In-Services as well as all required education/in-services as per organizational requirements.
- Other duties as assigned.
EDUCATION AND EXPERIENCE:
- Completion of an accredited Licensed Practical Nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).
- Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation.
- Experience working in a Supportive Living environment.
- Harm Reduction experience or education will also be considered an asset.
- A team player with excellent interpersonal, communication and organizational skills.
- Basic computer knowledge and a minimum typing speed of 30 w.p.m
- Experience with RAI MDS 2.0 HC as well as Point Click Care (PCC) is highly preferred.

This industry requires empathy for and an understanding of the needs of seniors and requires a complete and current (within three months) Vulnerable Sector Check including a Criminal Background Check.
Fast paced, challenging, working environment that provides/coordinates complex services to seniors and other marginalized populations. The ability to be flexible to meet the demands of this work environment is required for success. An understanding of Addictions and Mental Health and Trauma Informed care, in a Harm Reduction philosophy is an asset. This work can be stressful, and you may encounter challenging behaviors including physical or verbal aggression. This position does require shift work.
