Position Summary
In partnership with other members of the medical staff, the hospitalist will provide care for designated hospital in-patients. They may be most responsible physician (MRP) for these patients or involved in concurrent care along with one of the specialists. The hospitalists will work with the multidisciplinary team to provide care and services at Orillia Soldiers’ Memorial Hospital (OSMH) in accordance with accepted and evolving standards of clinical practice. They will utilize excellent diagnostic and therapeutic skills for the assessment and management of a wide range of medical conditions.
RESPONSIBILITIES:
The primary responsibility is for the provision of the highest quality of medical care, utilizing the resources of OSMH in a prudent manner, and working within their scope of training, experience and skill level.
The hospitalist will be responsible for:
• Providing MRP coverage for designated in-patients (designated patients are “unattached”- out of area or not currently enrolled with a physician or group that does inpatient care)
• Providing concurrent care when requested by a specialist based on complexity
• Participating in the provision of 24/7 coverage with the other hospitalists
• Participating in the multidisciplinary approach to clinical utilization:
• Communicating effectively with other members of the care team and the other hospitalists in order to facilitate transfer of patient care
• Ensure that reviews, meetings and consultations with team, patients and families are daily, regular and ongoing.
• Actively participate as a preceptor for medical trainees to develop inpatient acute care skills.
• Ensure that case conferences for complex multi-morbid patients include primary care and receiving agencies.
• Communicate discharge plans to the receiving community physician or nurse practitioner in a manner that optimizes continuity of care. (eg. discharge summaries will be dictated on the day of discharge)
• Use the Electronic Medical Record to its full capacity, seeking additional training as required.
• Consistently use available order sets electronically as appropriate.
• Participate in education to support accurate and comprehensive coding practices.
• Develop competency with Dragon Medical One voice transcription when available.
Exemptions:
The Acute Care Hospitalists may opt “in” to the rotation for providing normal new-born care with current applicable training and experience, however, it is not as assigned duty.
Availability:
• Work a full seven day or a split week (model TBD), providing 24x7x365 coverage for the defined population of inpatients.
• Dedicate the work day to hospital work and expect to have a census of approximately 20 patients (appropriate census and over-census procedures to be negotiated)
• Have alternate arrangements with their group or team to cover community practice responsibilities if applicable
• Be available for admissions, timely rounding and effective discharges for patients attend daily bullet rounds and huddles on units where assigned patients are admitted
• Be readily available and accessible to patients, families and staff to create and continuously reassess the care plan together with the patient.
• Participate in Structured Interdisciplinary Bedside Rounding(SIBR) where implemented which includes patients and families as part of the care team.
• On-call availability 24/7 for new admissions and in-patients in association with other assigned hospitalists
Utilization and Quality:
• Uses resources efficiently and works to achieve benchmarks in quality, access, turn around times, etc. as determined by the department, Medical Advisory Committee and hospital
General:
• Behaves in a manner that supports the Mission, Vision and Values of OSMH.
Performs other duties as may be assigned by the Lead, Acute Care Hospitalist or Chief of Family Medicine.
Qualifications
• College of Physicians and Surgeons of Ontario (CPSO) unrestricted license that allows the physician to fulfil the requirements of this position.
• ACLS Training
Experience:
• Previous experience preferred
