West Champlain Family Health Team

The West Champlain Family Health Team (WCFHT) consists of a group of interdisciplinary health care providers and administrative staff who support the family physicians of the Family Health Organization (FHO). Our team FHT includes nurse practitioners, registered nurses, social workers, dietician, respiratory therapist, and chiropodist.  Collaboratively, our team provides a broad spectrum of chronic disease management, disease prevention and health promotion programs.
