Upper Canada Family Health Team

The Upper Canada Family Health Team(UCFHT) is composed of 27 physicians who work in groups of two or three over 10 different sites serving approximately 36,000 patients. The Brockville FHO is composed of 17 physicians spread over 7 sites in Brockville, serving approximately 25,000 patients. Working collaboratively within the team are allied health professionals and nurse practitioners providing primary care and programs.  As a member of the Brockville FHO, you and your patients would have the support of various programs and services offered by the UCFHT, which include:

Lung Health including Smoking Cessation
